I am quoting a Holy Text, such as the Bible or the Koran. 
The Bible or Koran is quoted by book, chapter number, a colon, and verse number.  After this, the translation should follow, either spelled out or in abbreviation. 

"For God so loved the World that he gave his only son, that those who believe shall not perish but have eternal life."  Luke 5:25

Popular translations and their abbreviations:
KJ = King James
NKJ = New King James
NIV = New International Version
NAS = new American Standard

I am quoting a play, such as Shakespeare
The quote is typically given with:
1) the name of the play in Italics, a comma;   
2) The act, capitalized and with a Roman numeral, a comma;
3) the scene, not capitalized and with a Hindu-Arabic numeral
